SUPPORT COORDINATION
Our dedicated Support Coordinator team member will help you understand your NDIS plan, activate it and work with other people in your circle of support (family, friends, service providers, therapists
and community and mainstream support such as doctors) in order to meet the goals of your unique NDIS. We will be there with you throughout your first plan and assist you to prepare for your next plan. Our friendly and reliable team are here to answer all your questions.
ASSISTANCE WITH SOCIAL & COMMUNITY PARTICIPATION
We provide professional disability support services. Our experienced Case Manager will understand your individual plan goals. We provide tailored support services to meet your needs and preferences. As a team, we will arrange a support worker to assist you work towards your goals such as support with grocery shopping, medical appointments, sporting and recreational events, social activities, accessing community facilities, visiting or making new friends, travel and transport, support to pursue hobbies and interests, and support in building confidence and social skills. SELF-CARE ASSISTANCE
Our professional support workers can assist you with your self-care hygiene routine to allow you to feel refreshed and confident every day. Some of these supports can include help with dressing, showering, and toileting.
LAWN MAINTENANCE
Our reliable and professional contractor can provide you with lawn maintenance, edging and trimming, and much more. We provide tailored services to meet your needs and budget. Whether you want a one-off tidy up lawn maintenance service or regular weekly, fortnightly or monthly maintenance visits, our team is happy to assist you.
HOUSEHOLD TASKS
Our dedicated team can support you to achieve your goals to live life independently. We provide a range of in-home support services such as meal preparation and delivery, writing a shopping list, house maintenance, cleaning bathroom and kitchen, vacuuming, help with laundry.
